If you get Extra Help from Medicare to help pay for your Medicare prescription drug plan costs, your monthly plan premium will be lower than what it would be if you did not get Extra Help from Medicare. This notice provides information on plan premiums for those eligible for the low-income subsidy.
Advance Directives are important legal documents that tell your loved ones and health care providers what kind of care you want to receive if you become seriously ill or injured. This notice – as well as Chapter 8 of the Member Handbook – provide more information about advance care planning as well as links to important legal documents.
Use this form if you want to choose someone to act on your behalf regarding claims, coverage decisions, appeals, or grievances. By signing this form, you authorize the representative to talk to Neighborhood, make requests, and get information about your case. You must give us a copy of the signed form.
