Neighborhood Has Resources for Patients to Help Them Manage Their Health

We realize that health-related questions or concerns may arise when you are unavailable. We’re here to help. At Neighborhood, we have resources such as our Nurse Advice Line to help provide patients the information they need to make an informed decision. The Nurse Advice Line is staffed by caring Registered Nurses that are available 24 hours a day, 7 days a week to help answer your patients’ questions about health concerns, injuries, medications and/or understanding their treatment options.

We believe that having access to resources, education and self-management tools can lower a patient’s health risks. Neighborhood also has an online tool available to help our members manage their health, called My Neighborhood Health Guide. When patients complete a Health Risk Assessment online, a Neighborhood Care Manager may call and offer to help them manage their health. Our team outlines key steps and awareness programs that your patient can take to reduce health risks. We encourage our members to share this information with you during their next visit.

This tool is for all Neighborhood members and is available in English and Spanish. They will need their Neighborhood ID number to register.
