Neighborhood Care Management

Eligible Neighborhood INTEGRITY  members will have a care manager assigned to be part of their health care team. Neighborhood INTEGRITY care managers help our members coordinate their health care needs to ensure that all medical, behavioral health, substance use, long-term services and supports (LTSS), social supports, and transportation needs are met. Care managers educate, coordinate, and assist with these services and include them as part of a member’s overall care plan.

What is a care manager?

Neighborhood care managers are qualified individuals who help you manage all of your providers and health care services. They will work with you and your care team to make sure you get the care you need to stay as healthy as possible.

Your Neighborhood care manager can:

  • Educate you about your health conditions, medications, treatments, and available resources in your community.
  • Inform you about available benefits and services, help address any medication issues, create your care plan, and work with you on setting health-related goals.
  • Help you coordinate and facilitate the medical services that you need including working with your providers to help keep you on track with your plan of care.
  • Help you find specialists and get any durable medical equipment (DME) you may qualify for.
  • Assist with the communication between all of the providers that may be involved in your care.

You can contact your care manager by calling 1-844-812-6896 (TTY 711), 8 a.m. to 8 p.m., Monday – Friday; 8 a.m. to 12 p.m. on Saturday. On Saturday afternoons, Sundays and holidays, you may be asked to leave a message. Your call will be returned within the next business day. This call is free.

How can you change your care manager?

If you feel like your care manager is not meeting your needs, you can ask to change to a new care manager. To request a change, call Member Services at 1-844-812-6896 (TTY 711), 8 a.m. to 8 p.m. Monday – Friday; 8 a.m. to 12 p.m. on Saturday. On Saturday afternoons, Sundays, and holidays, you may be asked to leave a message. Your call will be returned within the next business day. This call is free.

Neighborhood Health Plan of Rhode Island is a health plan that contracts with both Medicare and Rhode Island Medicaid to provide the benefits of both programs to enrollees.
