Effective October 1, 2016 Claims Reconsideration will no longer be accepted on the PGP grid. In the interim all Claims Reconsideration should be submitted to Neighborhood via mail with the Claims Reconsideration form and any documentation to justify the request. We are currently in the development stage of updating an electronic submission process moving forward. The Claims Reconsideration form can be found on the Neighborhood website located in the Provider Manual Forms menu.
