In accordance with recently updated program parameters, effective immediately, nursing home discharge to home is now a covered service under the transportation benefit for Rhode Island Medicaid members administered through MTM. All existing guidelines pertaining to advance booking and level of need will apply to this transportation service. This coverage does not apply to members that are in a Medicare skilled status.

Transportation can be arranged by utilizing MTM’s facility portal located at: https://mtm-prod.revealservices.net/www/link/#/login  or by calling 855-330-9131.
