For Eligible Medicare-Medicaid INTEGRITY Members

If you choose, you’ll have a care manager to help coordinate your care. The Neighborhood INTEGRITY care managers coordinate the care team. Your care manager will ensure your medical, behavioral health, substance use, long-term services and supports (LTSS), social and transportation needs are coordinated. Your care manager will coordinate these services and include them in your care plan.

Your care manager will also:

  • Provide education about conditions, medications, treatment and resources in the community.
  • Provide education on available benefits and services, medication adherence, self-management and goal setting.
  • Coordination and facilitation of medical services and assistance in adherence to the member's plan of care with their provider.
  • Provide assistance with access to specialists and durable medical equipment (DME) providers. Enhancing communication between multiple providers that may be involved in the individual's care.

What is a care manager?

A care manager is a qualified individual who helps you manage all of your providers and services. He or she works with your care team to make sure you get the care you need.

Neighborhood Health Plan of Rhode Island is a health plan that contracts with both Medicare and Rhode Island Medicaid to provide the benefits of both programs to enrollees.
