*This plan is a type of Medicare Advantage plan specifically designed for individuals eligible for both Medicare and Medicaid (dual eligibles). These plans provide Medicare-covered services and are required to coordinate these benefits with the enrollee's Medicaid benefits.

Neighborhood Dual CONNECT is a health and drug plan for Rhode Islanders eligible for both Medicare and Medicaid and are Qualified Medicare Beneficiaries (QMBs). Whether you are caring for yourself or a loved one, we can help you get the care and support you need.

Please call us at 1-844-812-6896 (TTY 711), 8:00 a.m. to 8:00pm, seven days a week from October 1 through March 31. From April 1 through September 30 you can call us 8:00 a.m. to 8:00 p.m. Monday through Friday. You may leave a voicemail on Saturdays, Sundays, and federal holidays.

On Saturday afternoons, Sunday, and holidays, you may be asked to leave a message. Your call will be returned within the next business day. The call is free.
How can I replace my member ID card? Expand/Collapse Toggle

If you need a new Dual CONNECT member ID card, please call Member Services at 1-844-812-6896 (TTY 711). A Member Services representative can help you replace your card.

You may also ask for a new card online at this page .


How can I disenroll from the plan? Expand/Collapse Toggle

You may leave Neighborhood Dual CONNECT at any time during the year by calling us at 1-844-812-6896 (TTY 711). You may also call Medicare at 1-800-MEDICARE (1-800-633-4227). TTY users (people who have difficulty with hearing or speaking) should call 1-877-486-2048. When you call 1-800-MEDICARE, you can also enroll in another Medicare health or drug plan. If you leave Neighborhood Dual CONNECT, your coverage will end the last day of the month after you tell us. If you leave Neighborhood Dual CONNECT and don’t join a Medicare health or prescription drug plan, you’ll be covered under Original Medicare and Medicare will enroll you in a Medicare prescription drug plan. You will get your Medicaid benefits directly through Rhode Island Medicaid Fee for Service (FFS).
