What is a Dual Special Needs plan (D-SNP)?

A Dual Special Needs plan is also known as a D-SNP. This type of health insurance plan is for people who are eligible for both Medicare and Medicaid. A D-SNP plan is designed for those who need more help due to disabilities, age, or health conditions. Neighborhood has two D-SNP plans: INTEGRITY for Duals (HMO D-SNP) and Dual CONNECT (HMO D-SNP).

What is Neighborhood INTEGRITY for Duals?

INTEGRITY for Duals is Neighborhood’s fully integrated dual-eligible special needs plan – the only one in Rhode Island! This means that Neighborhood coordinates your Medicare, Medicaid, and Part D benefits.
