Working Together to Help INTEGRITY Members Reduce the Risk of Falls

Do you screen INTEGRITY members for risk of falling during their annual visit? If not, you might consider asking some basic questions. According to the 2011 American Geriatrics Society /British Geriatrics Society, a fall risk assessment can include: reviewing history of falls, review of medications, gait, balance, and mobility testing; and visual acuity tests.
If you determine that a member is at risk, besides encouraging exercise and physical activity, please contact Neighborhood’s Case Management department to consider arranging for a Physical Therapy Evaluation or Home Safety Assessment.
