Care Coordination: Your Health, Your Team

Sometimes staying healthy is a team effort. Depending on what medical conditions you may have, you might need help from more than one provider. Because health care can be complicated, you might feel overwhelmed by the different medications and instructions you receive from providers. That’s okay. Neighborhood wants to make sure that you can get well and stay well, no matter your medical needs, and that you feel safe and comfortable.

You can sign up for Neighborhood’s Case Management Program. We will then give you a personal case manager to coordinate your care if you need one.  

Personal case managers are nurses or social support staff who can help you make a plan for care that fits your unique needs. We know that dealing with long-term or multiple health issues can be stressful, and our personal case managers are here to help.

Your case manager will:

  • Be available Monday through Friday from 8:30 a.m. to 5 p.m.
  • Help you get the medical, social, behavioral and long-term care services you need.
  • Work with you, your family, and other providers to make sure all of your needs are met.
  • Help you find resources that might make life a little easier.

When you ask questions, we listen. We want to know what goes on with our members – we care about the details. Through our special health and wellness programs, Neighborhood can help you manage every part of your mental and physical health.

1. Mental health or substance abuse
Beacon Health Strategies is our behavioral health partner. Our members can call Beacon at 1-800-215-0058 (TDD/TTY 1-866-727-9441) 24 hours a day, seven days a week for confidential help. You don’t need a referral.

2. Disease management
If you are a patient with a chronic condition like asthma, diabetes or chronic obstructive pulmonary disease (COPD), our Disease Management program can help you live a healthy and active life. This program:

  • Keeps you informed
  • Lets you know what tests, medications, nutrition and other steps you can take to stay healthy
  • Gives you tips on what to ask your provider
  • Gives you access to nurses who can help you manage your condition

3. Prescription management

Providers and pharmacists don’t expect you to understand everything about medications. Our Pharmacy Home program helps members who take many prescriptions make sure they are taking them safely, and keeps all your prescriptions at only one pharmacy. You will receive a letter in the mail if you can sign up.

4. Tobacco cessation
Smoking is always harmful to your health, especially if you have other health issues like asthma or heart disease. Our Quit for Life program is designed to help you kick tobacco once and for all. Members who enroll in the program get access to health and wellness specialists, support groups and educational tools.
