Welcome to Take a Breath: Neighborhood's Chronic Obstructive Pulmonary Disease Program

Chronic conditions such as Chronic Obstructive Pulmonary Disease (COPD) can be difficult to manage. Neighborhood's Take a Breath program can help.

Take a Breath is a program to help Neighborhood members with COPD live active, healthy lives. Once enrolled in Take a Breath, you will get information about how to manage your COPD better.

If you do not appear to have your COPD well controlled you will get a call from a Neighborhood Care Manager who will offer you more information and support. If you would like, they can work with you and your doctors to help you take care of your condition and discuss ways to help you stay well. They can also help you with things that make it difficult to stay healthy and things that can make your COPD worse.
