Neonatal

The goal of the Neonatal Case Management program is to improve overall health and wellness and maximize quality, cost-effective outcomes for members who have been discharged to home from a Neonatal Intensive Care Unit (NICU) and who have who have been identified as being at risk due to complex medical and/or social needs. 

Participants include any member under the age of one who meets program admission criteria, generally those most at risk neonates who required a NICU stay.

The Neonatal Case Management program can perform such interventions as:

  • Coordination/facilitation of complex home care and DME needs;
  • Reinforce the provider plan of care;
  • Education for the primary care givers;
  • Addressing social concerns, such as adequate, safe housing and transportation.
