Neighborhood and RIte Care members now have to show citizenship and identity documents when they recertify.



         

Pre-natal (Bright Start)

The goal of the Bright Start Care Management program is to improve birth outcomes of children born to members by increasing the frequency and quality of prenatal care to high risk and minority populations.  A Health Risk Assessment (HRA) is used to stratify individuals into risk levels.

Participants include any high or moderate risk pregnant member as identified by a health risk assessment tool, physician referral or member self referral. Participants include those with medical and/or social risks.

The Bright Start program can perform interventions such as:

  • Support plan of care and coordination of needed services;
  • Pregnancy related education, including; nutrition, importance of prenatal care and post partum follow-up, pre-natal vitamins, and future birth control methods;
  • Facilitation of smoking cessation programs;
  • Addressing social issues, such as; domestic violence, housing, transportation and DCYF;
  • Provide community resource assistance to needy members.
