The Children with Special Health Care Needs Care Management program can help to improve a child’s health and wellness if they are at risk due to complex medical and/or social issues.  This program is for children under the age of 21.

How can we help you and your family?

• Can set up transportation for appointments
• Can help set up doctor appointments
• Can refer your child for help if they are depressed or having emotional problems
• Can help set up any services needed at home, including medical equipment.
• Can help refer you to community resources to address social concerns including food, housing, domestic violence.
