The Neonatal Case Management program can help you take care of your baby after your baby is sent home from the Neonatal Intensive Care Unit.

How can we help you and your family?

  • Can set up your baby’s needed visiting nurses and/or medical equipment;
  • Can help teach you how to care for your baby at home and when to call your doctor;
  • Can address social concerns, such as adequate, safe housing and transportation.
