Care Management Programs

If you have a complex health concern, we have care managers who can support you and your doctor.  The case managers will help you to set goals to improve your health and to develop a plan of care that helps you to meet those goals.

We can help you arrange all care, including;

• Doctor and specialist care
• Home Health Services (such as physical, speech, and occupational therapies; nursing & home health aides)
• Medical equipment and supplies
• Hospital care
• Mental health and substance abuse services
• Medication
• Working with you on social issues, including; transportation, housing and community resources
