About Communities of Care
Neighborhood is implementing a new program for its Medicaid enrollees called Communities of Care, as part of our contract with Rhode Island’s Medicaid Program. It was designed for members who have four or more emergency room (ER) visits in a 12-month period. The goal of Communities of Care is to improve health outcomes by helping members address their health care needs and in the process reduce avoidable or inappropriate emergency room use. The program also provides member incentives for participating and enhanced supports through the use of Peer Navigators and Care Managers. For a more detailed program description, please refer to this fact sheet developed by the Department of Human Services. Neighborhood will send all Dedicated Providers a provider communication form with member detail when a member has been assigned. Neighborhood will also provide supportive services as needed to the members enrolled in the Communities of Care Program, including access to behavioral health and medical case managers, as well as peer navigators. You and/or your patient should please call the following numbers as needed for supportive services:
