RIte Care

The RIte Care Case Management Program can help adults and children improve their health and wellness.

Participants in this program include children up to age 19 and parents with children under the age of 18.

Neighborhood Care Management can help you and your family in the following ways:

• We can help set up doctor appointments.
• We can assist members to work with customer service to arrange medically necessary transportation.
• We can help you set up doctor appointments with primary care or specialty doctors.
• We can work with you to refer to Beacon Health Strategies, our behavioral health partner, if needed.
• We can help you to coordinate services ordered by your doctor that are medically necessary for your home.
• We can refer you to community resources to assist you with social issues, including food, housing, and utilities.
