The Children with Special Health Care Needs (CSN) Care Management Program can help to improve a child's health and wellness if they are at risk due to complex medical and/or social issues.

This program is for children up to the age of 21.

Neighborhood Care Management can help you and your family in the following ways:

• We can help set up doctor appointments.
• We can assist members to work with customer service to arrange medically necessary transportation
• Neighborhood can help set up any services needed at home, including medical equipment.
• We can help you set up doctor appointments with primary care or specialty doctors
• We can work with you to refer to Beacon Health Strategies, our behavioral health partner, if needed
• We can help you to coordinate services ordered by your doctor that are medically necessary for your home
• We can refer you to community resources to assist you with social issues, including food, housing, and utilities
