Helping You and Baby after NICU

Neighborhood's Neonatal Case Management Program can help you coordinate and manage the many needs of your baby after being discharged from the Neonatal Intensive Care Unit (NICU).

• Neighborhood can help you coordinate services that are medically necessary, including visiting nurse service or medical equipment
• We can provide you ongoing education in caring for your child
• We can refer you to community resources to assist you with social issues, including food, housing, and utilities
• We can assist members to work with customer service to arrange medically necessary transportation
