Many over-the-counter (OTC) drugs are covered for RIte Care members with a prescription from their primary care practitioner (PCP). See the Neighborhood Formulary for the current list of covered OTC drugs.

         

Diabetes Program

The goal of the Neighborhood Diabetes Program is to help members with diabetes achieve and maintain good health outcomes by confident management of their diabetes.  We work with members and their doctors

  • To improve outcomes for Neighborhood members with diabetes by encouraging adherence to guidelines for adult diabetes care
  • To identify and assist newly diagnosed diabetics
  • To encourage improved patient self-management resulting in a decreased incidence of acute diabetic crises and long-term diabetes co-morbidities.

The program is designed for adult (18 and over) members with Type I or Type II diabetes, as defined by the following claims-based criteria for a given 24 month period:

  • Picked up insulin or oral hypoglycemics/antihyperglycemics (except glucophage/metformin) during the period and/or
  • Had two outpatient visits or one ER or inpatient visit with a diagnosis of diabetes during the period.

What Providers Receive

Results

  • 86% of members with diabetes have at least an annual Hb A1c test, an important indicator of diabetes control
  • 82% of members with diabetes have their lipid levels checked annually
  • An increasing percentage of members with diabetes have good control of HbA1c levels and lipid levels.
