Neighborhood wants to help our members stay healthy! Our goals are to: - Keep members well through health care check-ups;
- Help members learn healthy habits;
- Get members good care when pregnant;
- Work with members to manage their asthma or diabetes; and
- Assist members in finding the information they need to make good health care choices

Neighborhood provides Care Coordination and Case Management services for individuals with special healthcare needs. (link to: Asthma, Adult, Bright Start, Neonatal, Pediatrics, Children with Special Health Care Needs, Substitute Care, Connect CARRE). A Care Coordinator/Case Manager is a nurse or social worker who follows member cases as they go through the health care system. The purpose of a Care Coordinator/Case Manager is to have one person be the helpful partner.
