Connect CARRE

The goal of the Connect CARRE program is to improve the wellness and functional status of participants and maximize quality cost effective outcomes for high risk, adult Medicaid recipients with multiple chronic medical conditions.

Participants include Medicaid recipients who are 22 years of age and older with chronic medical conditions who meet the eligibility criteria. Participants include those with high utilization of acute care facilities or those who are at risk for recurrent adverse medical events.

The Connect CARRE program can perform such interventions as:

  • Initial and annual home assessments performed by the Nurse Care Manager
  • address issues relating to medication adherence
  • reinforcement of  the provider plan if care.
  • Facilitating communication between medical providers, behavioral health services, and community support services
  • Ensuring a medical home for each participant
  • Increasing the ability of participants and families to manage their care
  • Addressing social concerns such as housing and transportation
  • Identifying and coordinating services in the community setting to assist consumers in maintaining wellness
  • Facilitating referrals to community resources
  • Educating participants regarding their chronic disease
  • Encouraging consumers to adhere to self management goals
