Substitute Care Program

The goal of the Substitute Care Case Management Program is to provide for coordinated health care delivery system in which care is integrated, services are targeted to meet a child’s individualized needs, and access to care is timely.

Participants are those in the State’s foster care program. Neighborhood is the only insurer to provide case management services to this group of children.

The Substitute Care Case Management Program can perform interventions such as:

  • Perform an initial health screen to determine acuity and complexity of child’s needs
  • Care Coordination with a Social Care Coordinator
  • Intensive Care Management with a Registered Nurse
  • address issues relating to medication adherence
  • reinforcement of  the provider plan if care.
  • Coordination of Care with all Providers
  • Coordination with Biological Parents, Foster Parents and DCYF Case Workers
  • Coordination of medical and behavioral health services delivered by primary care physicians, specialty care physicians, mental health clinicians and other health care practicioners
  • Collaboration with CEDARR Family Centers
