Medically necessary Weight Management Programs are a covered benefit for RIte Care members.

         

The goal of the Children with Special Health Care Needs Care Management program is to optimize the efficient use of the health system resources in order to ensure positive patient outcomes in a cost effective manner.  The program aims to provide for a coordinated health care delivery system in which care is integrated, services are targeted to meet a child’s individualized needs, and access to services is timely.

Participates include any member, up to the age of 21, with complex medical and/or behavioral health needs and who qualify for Medicaid due to Supplemental Security Income, a “Katie Beckett” provision or are under Rhode Island’s adoption-subsidy program, as determined by the Department of Human Services.

The Children with Special Health Care Needs program can perform such interventions as:

  • Address issues relating to medication adherence
  • Reinforcement of  the provider plan if care
  • Facilitation and coordination of:
    • Transportation
    • Behavioral health
    • Provider appointments
    • DME
    • Home Health
       
  • Address social concerns:
    • Domestic violence
    • DCYF
    • Housing
    • Food
    • Community resources 

The case managers also address issues relating medication adherence and will work to reinforce the provider plan of care.
