The goal of the Children with Special Health Care Needs Care Management program is to optimize the efficient use of the health system resources in order to ensure positive patient outcomes in a cost effective manner. The program aims to provide for a coordinated health care delivery system in which care is integrated, services are targeted to meet a child’s individualized needs, and access to services is timely. Participates include any member, up to the age of 21, with complex medical and/or behavioral health needs and who qualify for Medicaid due to Supplemental Security Income, a “Katie Beckett” provision or are under Rhode Island’s adoption-subsidy program, as determined by the Department of Human Services. The case managers also address issues relating medication adherence and will work to reinforce the provider plan of care. Referrals to the Children with Special Health Care Needs Care Management program can be made by contacting a CSN Case Manager at 1-800-459-6019. |
