Neighborhood coordinates interpreter services for our members. Simply fax an Interpreter Services Request form to Neighborhood three (3) business days in advance of the scheduled appointment.

         

Pediatric
 
The goal of the Pediatric Care Management program is to improve overall health and wellness and maximize quality, cost-effective outcomes for pediatric members determined to be at risk due to complex medical and/or social needs.

Participants include any member under the age of 19 who meets program admission criteria. Participants include those with chronic and/or catastrophic diagnoses.

 The Pediatric Care Management program can perform such interventions as:

  • Address issues relating to medication adherence
  • Reinforcement of  the provider plan if care
  • Facilitation and coordination of:
    • Transportation
    • Behavioral health
    • Provider appointments
    • DME
    • Home Health
       
  • Address social concerns:
    • Domestic violence
    • DCYF
    • Housing
    • Food
    • Community resources
       

The case managers also address issues relating medication adherence and will work to reinforce the provider plan of care.
